AIDS Foundation of Chicago
The AIDS Foundation of Chicago (AFC) was founded in 1985 with the mission to “lead the fight against HIV/AIDS and improve the lives of people affected by the epidemic.” AFC is a local and national leader in building and maintaining strong community collaborations, and provides an exemplary model of “lead agency” coordination of housing and other services for individuals affected by HIV/AIDS. The organization has been identified by the Kaiser Family Foundation as one of the key organizations in HIV/AIDS nationwide. Currently, AFC provides funding for various agencies, and oversees the coordinated services of more than 60 HIV/AIDS case management and housing providers throughout the Chicago area.
Prison Fellowship
Prison Fellowship (PF) is an organization that, through the teachings of Jesus Christ, is dedicated to helping prisoners and ex-prisoners get their lives back on track and restoring peace to our cities and communities endangered by crime. Their Transformational Ministry—a coordinated effort of values-based reentry programs and services developed by and affiliated with Prison Fellowship—is proving that prisoners can get out and stay out.
Utah Department of Veterans Affairs
The mission of the Utah Department of Veteran’s Affairs (UDVA) is to ensure access to benefits for all 166,000 war veterans residing in Utah. Until recently, though, UDVA staff possessed contact information for only about 11,000 of these, and Utah vets were, on average, claiming only half of the benefits claimed by veterans in other states. An outdated information system—one that relied on overflowing file cabinets and paper spreadsheets—posed significant challenges to efficiently providing services for deserving veterans.
YWCA of Salt Lake City, Utah
The YWCA of Salt Lake City has been dedicated to strengthening the safety, health, and well-being of local women and children since 1906. It serves more than 8,000 local women and children annually. The YWCA had an increasing need to track clients within and between programs more effectively and efficiently. “We had nine programs operating under nine different data management processes,” explains Keri Jones, Chief Program Officer.
